CONSENT FORM for Youth under the age of 18 years.

To inspire creativity and self-expression among Sahtú youth and adults during this unfortunate time of social distancing and isolation due to the COVID-19 pandemic, the Sahtú Renewable Resources Board (SRRB) and the Sahtú Youth Network (SYN) are launching a monthly photography & videography contest series. Contestants will submit a photo or video and answer some questions about how they are feeling and coping during these trying times.

The photos and videos will be a way to encourage youth to share their knowledge and their thoughts with others and a way for youth to connect around the Sahtú.

There are two categories for the contest, ages 13 to 17 and ages 18 to 30. Each month, there will be one grand prize awarded, and one prize for each age category. Each submission will be placed into a draw and the winner will be chosen at random from each category. The grand prize is an iPad and the age category prize is a Northern gift card.

Your child’s participation is voluntary. Once submitted, the photo or video will become public material and may be broadly circulated on social media. You may withdraw your child’s photo or video at any time prior to it being posted on social media and the Sahtú Youth Network website. As with any content posted online, there is a risk that your contribution may be reproduced by others on the internet in an undesirable manner. Your consent on this form acts as acceptance of and agreement to the terms found in the SYN Contest Rules. Per the SYN Contest Rules, the SRRB and SYN are not liable for any damages that occurs due to an entrant’s participation in the contest.

1. Eligibility: This Contest is open only to residents in the Sahtú region of the Northwest Territories who are between 13 to 35. If there are people younger than 18 years of age in your photo, you grant permission for the Sahtú Renewable Resources Board (“SRRB”)/Sahtu Youth Network (“SYN”) to use the photo as per Section 8 below (“Rights Granted by You”).

2. Agreement to Rules: By submitting a photo, the Contestant (“You”) agree to be fully, unconditionally bound by these Rules, and You represent and warrant that You meet the eligibility requirements. In addition, You agree to accept the decisions of the SRRB/SYN as final and binding as it relates to the content of this Contest.

3. Contest Period: Entries will be accepted online beginning April 10 – July 20, 2020.

4. How to Enter: The Contest must be entered by submitting a photo via e-mail to info@srrb.nt.ca, or by sharing the photo publicly on Facebook, Twitter, or Instagram using the hashtag #SYNSahtuStrong. The entry must fulfill all Contest requirements, as specified, to be eligible to win a prize. Entries that are incomplete or do not adhere to the rules or specifications may be disqualified at the sole discretion of the SRRB/SYN.

5. Prizes: Each month the contest is ongoing, the contest will have multiple Winners. There will be one grand prize awarded, as well as a prize for each age category. The prize for the Winner of each age category is a $150 Northern gift card. The prize for the Winner of the Grand Prize is an iPad. The specifics of the prize shall be solely determined by the SRRB/SYN. No cash or other prize substitution shall be permitted except at the SRRB/SYN discretion. Acceptance of the prize constitutes permission for the SRRB/SYN to use the Winner’s name, likeness, and entry for purposes of advertising and trade without further compensation.

6. Odds: The odds of winning depend on the number of eligible entries received.

7. Winner Selection and Notification: The Winner will be selected in a draw system. Each complete entry will be entered into a draw and a winner selected at random under the supervision of the SRRB/SYN. The Winner will be notified by message via Facebook, Twitter, Instagram, or email, depending on how the Winner entered the Contest, within one week following the close of the Contest. The SRRB/SYN shall have no liability for the Winner’s failure to receive notices due to spam, junk e-mail, or other security settings, or for the Winner’s provision of incorrect or otherwise non-functioning contact information. If the Winner cannot be contacted, is ineligible, fails to claim the prize within 10 days from the time the award notification was sent, the prize may be forfeited and an alternate Winner selected.

8. Rights Granted by You: By entering this content (e.g., photo, video, text, etc.), You understand and agree that the SRRB/SYN, anyone acting on behalf of the SRRB/SYN and the SRRB/SYN licensees, successors, and assigns, shall have the right, where permitted by law, to print, publish, broadcast, distribute, and use in any media now known or hereafter developed, in perpetuity and throughout the World, without limitation, your entry, name, portrait, picture, voice, likeness, image, statements about the Contest, and biographical information for news, publicity, information, advertising, public relations, and promotional purposes without any further compensation, notice, or consent. By entering this content, You represent and warrant that your entry is an original work of authorship, and does not violate any third party’s proprietary or intellectual property rights. If the content of your entry is claimed to constitute infringement of any proprietary or intellectual proprietary rights of any third party, You shall, at your sole expense, defend or settle against such claims. You shall indemnify, defend, and hold harmless the SRRB/SYN from and against any suit, proceeding, claims, liability, loss, damage, costs or expense, which the SRRB/SYN may incur, suffer, or be required to pay arising out of such infringement or suspected infringement of any third party’s right.

9. Limitation of Liability: By entering the Contest, You agree to release and hold harmless the SRRB/SYN and its subsidiaries, affiliates, advertising and promotion agencies, partners, representatives, agents, successors, assigns, employees, officers, and directors from any liability, illness, injury, death, loss, litigation, claim, or damage that may occur, directly or indirectly, whether caused by negligence or not, from: (i) such entrant’s participation in the Contest and/or his/her acceptance, possession, use, or misuse of any prize or any portion thereof; (ii) technical failures of any kind, including but not limited to the malfunction of any computer, cable, network, hardware, or software, or other mechanical equipment; (iii) the unavailability or inaccessibility of any transmissions, telephone, or Internet service; (iv) unauthorized human intervention in any part of the entry process or the Contest; (v) electronic or human error in the administration of the Contest or the processing of entries.

10. Disputes: As a condition of participating in the Contest, You agree that any and all disputes that cannot be resolved between the parties and causes of action arising out of or connected with this Contest, shall be resolved individually. Further, in any such dispute, under no circumstances shall You be permitted to obtain awards for, and hereby waive all rights to, punitive, incidental, or consequential damages, including reasonable attorney’s fees, other than actual out-of-pocket expenses (i.e. costs associated with entering this Contest). You further waive all rights to have damages multiplied or increased.

11. Sponsor: The Sponsor of the Contest is the SRRB/SYN through the support of MACA, Tides, Lawson Foundation.

12. By submitting a photo, You, the Contestant, have affirmatively reviewed, accepted, and agreed to all of the above Rules.
